How your Personal Information is used and stored
I will keep our conversations confidential unless there are exceptional circumstances where I'm either required by law to break confidentiality, or if I'm concerned that:
- you're in immediate danger
- you are a danger to yourself or others
Your notes are kept in hard copy in a locked cabinet and at my home address, or in secure storage for the length of time recommended by my insurer.
I will forward a copy of your notes should you require one. I reserve the right to charge a small administration fee for this.
The clinic/ assistants, only have access to your name and contact details, not your medical notes. They may contact you to alter appointments. They may also post your herbs or handle your already dispensed medicines.
Your medical notes may be passed on to a locum with your written permission in the event that I am unable to work for a significant period of time.
You will be asked at your first consultation how you would like me to contact you whether by telephone, email, text or in writing. If at short notice I need to contact you to cancel an appointment I will use any method I have available to make you aware of this to prevent you making a wasted journey.
You will be asked during your first consultation whether you wish to receive occasional emails about events that I may lead or relevant news that I wish to share. You are free to unsubscribe from these at any time.
I will not share your contact details with any other companies for marketing purposes.
Your name and any payments amounts to me are logged in my computer for my personal accounting records only.
All computers and mobile devices are password protected.
This privacy policy may be reviewed or updated at any time.
